The bar doesn't show on my store

Turn on the "Free shipping bar" app embed: Online Store → Themes → Customize → App embeds, then save. The app's setup guide has a button that does this.
Check that the bar is on in the app's Settings.
The bar only shows in countries with a free shipping amount. Open the app to see the amount per country: add a free shipping rate with a minimum order price, or an automatic free shipping discount, for the countries that miss one.
By default the top bar shows on product pages. You can choose other pages in the app embed's settings.

The amount is wrong

The app reads the amounts from your default shipping profile and your automatic free shipping discounts, and updates when they change. Click "Sync shipping rates" in the app to read them again. On Pro you can also set your own amount per country.

The progress card isn't in my cart

The card places itself on the cart page and in the cart popup or drawer of Shopify's own themes (such as Dawn and Horizon). In other themes, add the "Free shipping progress" block where you want it in the theme editor.

Plans

Free has the top bar, the progress card and the automatic amounts in 14 languages. Pro adds product suggestions, your own texts and your own amounts per country. Upgrade, downgrade or cancel in the app at any time; settings you made on Pro are kept when you switch.
